We are seeking a dedicated and adaptable On-Call Hospice Nurse to deliver compassionate nursing care in response to patient needs during after-hours and on an as-needed basis. The ideal candidate will possess extensive clinical experience and demonstrate a strong commitment to patient-centered care. This role offers an opportunity to provide vital support to patients and families during challenging times while working within a collaborative healthcare environment. The Fisher Home serves patients in Hampshire, Hampden, and Franklin counties.
*Responsibilities*
* Respond promptly to after-hours patient calls, providing expert assessment and guidance to ensure comfort and safety.
* Conduct thorough patient assessments, including vital signs, symptom management, and physiological evaluations.
* Collaborate effectively with healthcare professionals, including physicians, social workers, and other team members, to coordinate patient care plans.
* Manage electronic health records (EHR) documentation accurately and efficiently in compliance with HIPAA regulations.
* Provide education and emotional support to patients and their families regarding symptom management and end-of-life care.
* Maintain up-to-date knowledge of health regulation policies, infectious disease protocols, and hospice care standards to ensure compliance and quality assurance.
*Qualifications*
* Valid RN license in Massachusetts
* Prior experience in hospice or palliative care is highly desirable; experience with home health or post-acute care is a plus.
* Demonstrated proficiency in patient assessment, case management, and utilization management processes.
* Strong knowledge of medical terminology, physiology, and health regulation policies.
* Excellent communication skills with the ability to collaborate effectively within multidisciplinary teams.
* Familiarity with electronic health records (EHR) systems and HIPAA compliance standards.
* Ability to manage multiple priorities efficiently while maintaining compassionate patient service during on-call hours.
