Hartford Healthcare seeks a compassionate Weekend Hospice RN to provide skilled nursing care to patients in their homes across our service area. You will complete comprehensive assessments, manage symptoms and medications, and implement individualized care plans in collaboration with an interdisciplinary hospice team. This role requires strong clinical judgment, excellent communication, and the ability to support patients and families through end-of-life care. Weekend coverage offers schedule flexibility and the opportunity to practice autonomously with robust team support and resources.
Responsibilities
- Provide weekend hospice nursing visits in patient homes across the service area
- Complete comprehensive patient assessments and ongoing reassessments
- Develop, implement, and update individualized hospice care plans
- Administer medications, treatments, and symptom management interventions
- Educate patients and families on disease process, medications, and comfort measures
- Collaborate with physicians, social workers, chaplains, and aides to coordinate care
- Document all care in the EMR accurately and in a timely manner
- Respond to urgent patient needs and triage changes in condition
- Ensure compliance with hospice regulations, policies, and safety standards
- Participate in team meetings and contribute to quality improvement initiatives
