Hartford Healthcare seeks a compassionate Hospice Nurse Weekend to join our Healthcare & Medical Services team. In this field-based role, you will provide end-of-life nursing care, pain and symptom management, and emotional support to patients and families in their homes. You will complete assessments, administer medications, coordinate with an interdisciplinary team, and document care in the EMR. Responsibilities
- Provide weekend hospice nursing care in patients' homes, focusing on comfort and dignity at end of life.
- Conduct comprehensive assessments, manage pain and symptoms, and adjust care plans as needed.
- Administer medications and treatments per provider orders, following hospice protocols.
- Educate and support patients and families on disease progression, comfort measures, and available resources.
- Collaborate closely with physicians, social workers, chaplains, and aides to coordinate holistic care.
- Document all assessments, interventions, and outcomes accurately in the electronic medical record.
- Participate in on-call rotation and respond promptly to urgent patient and family needs.
- Contribute to quality improvement, safety initiatives, and a compassionate, team-based culture.
