Responsibilities
- Provide compassionate hospice care to patients and families in the Manchester area, following Hartford Healthcare standards and care plans
- Conduct comprehensive patient assessments, monitor symptoms, and communicate changes to the interdisciplinary team
- Coordinate with physicians, social workers, chaplains, and caregivers to deliver holistic, person-centered end-of-life care
- Administer medications and treatments, ensuring comfort, safety, and adherence to hospice protocols
- Educate patients and families on disease progression, comfort measures, and available support services
- Document care, assessments, and interventions accurately in the electronic health record
- Participate in interdisciplinary team meetings and contribute to individualized care planning
- Support bereavement and emotional needs of families with empathy and professionalism
