What You ll Do - Effectively evaluate, rehabilitate, and develop customized care plans for patients with physical functioning disorders and wounds
- Utilize standardized assessments and evidence-based practices to support clinical interventions
- Administer procedures including manual techniques, ambulation, therapeutic exercises, modalities, wound care, and assistive device support
- Monitor patient progress, adjusting treatments and documenting outcomes accurately within regulatory and clinical standards
- Communicate patient progress and collaborate with healthcare team members and families
- Complete detailed discharge summaries and maintain thorough records in electronic documentation systems
- Participate in meetings, trainings, and conferences as required
- Supervise and support physical therapist assistants, rehabilitation aides, and students when applicable
- Ensure compliance with APTA and state practice acts, adhering to the highest professional standards
What You Need - Valid MA Physical Therapist license
- At least 1 year of prior LTC or rehab experience preferred
- Strong clinical and communication skills
- Ability to work independently and as part of a team
- Commitment to providing compassionate, patient-centered care
