*Physical Therapist (PT) Duties and Responsibilities:*
* Adheres to CMS guidelines by providing initial and ongoing assessments to determine the level of functioning, including OASIS assessment at appropriate time points.
* Gathers background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient’s physical and psychosocial functioning.
* Develop and revise the plan of care in consultation with the physician and other care team members.
* Responsible for planning, evaluation, and treatment implementation of an assigned caseload.
* Preparing clinical progress notes and evaluating outcomes.
* Able to complete clinical documentation within 24 hours of patient contact.
* Supervising therapy assistants and home health aides as appropriate.
* Educate patients and/or families about proper safe use and recommend methods for the installation of adaptive equipment.
*Physical Therapist (PT) Qualifications and Skills:*
* Currently licensed by the Massachusetts Board of Physical Therapy (PT) Examiners.
* Experience in acute care settings or Home Health is preferred but not required.
* Able to maintain strict confidentiality about protected and sensitive information.
* Must be able to solve problems independently and confidently by applying analytical and logical thinking.
* Able to read, analyze, and interpret reports, general business directives, policy and procedure statements, and governmental regulations.
* Able to write reports and correspondence at a professional level.
* Able to effectively communicate with individuals from diverse backgrounds and varying educational levels.

Pay: $120,000.00 - $130,000.00 per year
