*Job Summary*
We are seeking a dedicated and compassionate Physical Therapist specializing in Home Care to provide exceptional patient-centered therapy services in clients' homes and community settings. The Physical Therapist provides skilled physical therapy services in the patient’s home, completes evaluations, develops care plans, and works collaboratively with our interdisciplinary team to ensure excellent outcomes and compliance with CHAP, CMS, and MassHealth guidelines.

*Responsibilities*

* Perform initial assessments, re-evaluations, and discharge assessments in accordance with CHAP and CMS regulations.
* Develop individualized, goal-directed treatment plans.
* Provide skilled interventions including gait training, strengthening, balance training, safety training, and functional mobility.
* Educate patients and caregivers on home exercise programs and safety measures.
* Collaborate closely with RNs, OTs, STs, MSWs, and HHAs.
* Document visits accurately and promptly in the EMR.
* Communicate changes in patient condition to the clinical team and physicians.
* Follow all MassHealth, Medicare, and commercial insurance guidelines for documentation and visit frequency.

*Requirements*

* Active MA Physical Therapist (PT) License (required)
* 2+ years of clinical experience, preferably in home care
* Strong understanding of CHAP standards and CMS Conditions of Participation
* Experience with OASIS (preferred)
* Reliable transportation for home visits
* Strong clinical judgment, communication, and documentation skills

Pay: $45.00 - $50.00 per hour
