With 114+ years of service to the community, the Natick Walpole Visiting Nurse Association is one of the most trusted non-profit providers of home health, public health, and community- based certified health home care in the region.
We are looking for a compassionate, reliable PT to join our team to provide excellent care and who enjoys working with patients in an at-home setting.
*Territory Includes: Natick, Ashland, Framingham, Wayland, Weston, Wellesley, Needham, Dover, Sherborn*
*Responsibilities*:
* Provide care and services in accordance with the plan of care.
* Assist community members and health field personnel to assess, plan for, and provide needed health and related services.
* Document care provided in an accurate and timely manner.
* Provides teaching to clients and families.
* Coordinate and collaborate with other members of the health care team, community agencies, and referral sources.
* Communicate the plan of care with patients and families and insures understanding of the exercise program prescribed.
*Requirements/Qualifications:*
* Current licensure from the Board of Allied Health in the State of Massachusetts
* Current valid driver’s license
* Graduate of an accredited college of physical therapy
* Must have demonstrated competence in community health or clinical patient care setting, providing direct patient care with patients for a minimum of two years. Associate's degree and graduation from an accredited Physical Therapy program.
*Skills/Competencies:*
* Demonstrates responsibility for self-direction
* Demonstrates the ability to learn and utilize computer technology
* Effective communication skills
* The ability to work in a team-oriented environment as well as independently
Job Type: Full-time
Pay: From $87,000.00 per year
